Participants Under the age of 18 must have a parent’ s or guardian’ s signature. 

Participants Under 16 years of age will not be allowed to participate.

Participant agrees that they have no mental or physical handicaps which interfere with them participating in the 2025 Polar Bear Dip.

Participant agrees that they are not under the influence of any drugs, alcohol, prescription drugs or other substance which impairs his faculties.

The participant agrees by submitting this form and paying that he or she, the undersigned for him or herself, personal representatives, heirs and next of kin, that I / we hereby release, waiver of discharge and further covenant not to sue and hold harmless, the Houghton Lake Chamber of Commerce, it’s Officers, Committee and organizations sponsoring Tip-Up Town, U.S.A., it’s affiliates, their employees, owners and anybody else participating in the above named event from any loss or damage, injury to myself or others, death to myself or other while participating in the above named event.
